“One important thing that is coming to the forefront is transition of care. The LA County Department of Health and the Orange County Department of Health are trying initiatives to get different facilities to communicate with each other for transition of care. Whether you go from acute care to long-term acute care (LTAC), or LTAC to nursing home, or nursing home to acute care, transferring information about susceptibly, resistance, and prior resistant isolates, and why people may or may not be on isolation or what they’ve had before, is really important to improve health care, and we need to all improve our communications. Transition of care with simple forms is really something that needs to be done in the future.
