Along with a 1 reported case in Massachusetts, the

New Hampshire Department of Health and Human Services (DHHS) has confirmed a person infected with eastern equine encephalitis (EEE) has died. The public health agency said the person was hospitalized due to severe central nervous system disease. The last reported human EEEV infection in New Hampshire was in 2014, when DHHS identified 3 human infections, including 2 fatalities. In the same statement, DHHS mentions there has been 1 confirmed case in Vermont this summer.

On August 16, the Massachusetts Department of Public Health (MDPH) released a statement discussing the confirmation of the first human case of EEE in the state this year. The confirmed case was discovered in an 80 year old man who was exposed to the vector borne disease in Worcester County. As a result, the risk level in the local communities of Douglas, Oxford, Sutton, and Webster has been raised to critical, and in Dudley, Northbridge, and Uxbridge the risk is increased to high.

Additionally, the neighboring communities of Auburn, Charlton, Grafton, Leicester, Mendon, Millbury, Millville, Southbridge, and Upton are now considered at moderate risk according to MDPH.

“This is the first time we have seen a person infected with EEE in Massachusetts since 2020,” Public Health Commissioner Robbie Goldstein, MD, PhD, said in a statement. “EEE is a rare but serious disease and a public health concern. We want to remind residents of the need to protect themselves from mosquito bites, especially in areas of the state where we are seeing EEE activity.

Though very rare, EEE is serious and a potentially fatal disease that can affect people of all ages. EEE is generally spread to humans through the bite of an infected mosquito. There were 12 human cases of EEE in Massachusetts in 2019 with 6 deaths, and 5 human cases with 1 death in 2020. There were no human cases of EEE in Massachusetts in 2021, 2022, or 2023.

EEE has a fatality rate of 33% to 70%, with most deaths occurring 2–10 days after the onset of symptoms.

In addition to a high mortality rate, long-term physical and mental complications, including permanent neurological deficits have been identified in many people who survive EEE.

Plymouth is approximately 40 miles south of Boston along the coast, and Worcester County is in the middle of the state, so the 2 areas are not in close proximity, but due to the elevated risk locally, the town of Plymouth decided to take action in response to outdoor parks. As of last Friday, all town public parks and fields are close daily from dusk until dawn.

“As Mass DPH has now elevated Plymouth’s EEE risk status to high, it is important to take extra precautions when outdoors and follow state and local health guidelines to avoid unnecessary risk to exposure to EEE. The health and safety of our community, residents, and visitors remain our priority,” Plymouth’s Commissioner of Health and Human Services, Michelle Bratti, said in a statement.

According to the Centers for Disease Control and Prevention (CDC) most people who are infected with the virus do not develop symptoms; however, for those that do, the date from the bite to the onset of the illness is 4 to 10 days. EEE can also result in fever or neurologic disease. The type of illness will depend on the age of the person and other factors. Those who are infected can develop meningitis.

There are no FDA-approved treatments for EEE and for patients who develop severe cases, they can be hospitalized to receive supportive treatment.

Over the weekend, MDPH detailed plans of spraying for mosquitos including aerially in areas of Plymouth County, and truck-mounted spraying in parts of Worcester County. As of Saturday, 10 communities in Massachusetts were raised to high or critical risk for EEE virus.

“The risk from EEE is high in parts of Plymouth County and critical in parts of Worcester County,” State Epidemiologist Catherine M. Brown, DVM, said in a statement. “In addition to recommending that people use mosquito repellent with an EPA-registered active ingredient and clothing to reduce exposed skin, we also recommend that evening outdoor events be rescheduled to avoid the hours between dusk and dawn. The mosquitoes most likely to spread EEE are most active during the dusk to dawn hours.”

FDA Approves Oral Sulopenem (Orlynvah) for Uncomplicated Urinary Tract Infections

The FDA has approved oral sulopenem (sulopenem etzadroxil/probenecid), marketed as Orlynvah from Iterum Therapeutics, based on data from a pair of phase 3 trials, SURE 1 (NCT03354598) and REASSURE (NCT05584657), demonstrating its effectiveness against infections caused by 

In the SURE 1 trial, oral sulopenem showed superiority over ciprofloxacin for fluoroquinolone-resistant infections, while the REASSURE trial indicated a better response rate compared with amoxicillin/clavulanate. Experts view this approval as a significant advancement in UTI treatment alternatives, especially amid growing concerns about antimicrobial resistance. The FDA's Antimicrobial Drugs Advisory Committee highlighted the need for careful post-market monitoring to prevent off-label use.

Approximately 40 million uUTI prescriptions are generated annually in the US, with about 1% because of pathogens resistant to standard oral antibiotics. Orlynvah is contraindicated in patients with a history of hypersensitivity to its components and may cause adverse reactions such as diarrhea and nausea.

Earlier this year, in September, the FDA's Antimicrobial Drugs Advisory Committee (AMDAC) met to discuss the drug's new drug application,

 focusing on the overall benefits and risks and key considerations for medical providers. The agency did not ask the AMDAC to vote on any matter in the meeting. Fishman noted at the time that the company was "encouraged by the AMDAC’s discussion" and pleased that the AMDAC considered it an important option for certain patients and agreed on its appropriate use.

Key Information From The SURE-1 and REASSURE Trials

Oral sulopenem was evaluated against ciprofloxacin in patients with uUTIs, focusing on quinolone-susceptible and nonsusceptible uropathogens. The study enrolled 1671 patients, randomly assigning them to receive either oral sulopenem or ciprofloxacin. In the quinolone nonsusceptible population, sulopenem demonstrated a significantly higher overall response rate of 62.6% compared with 36% for ciprofloxacin. 

In the ciprofloxacin-nonsusceptible group, clinical success was achieved in 83% of the sulopenem group versus 62.6% in the ciprofloxacin group, while microbiological success was noted in 74.1% of patients treated with sulopenem compared with 49.6% for ciprofloxacin, both with statistically significant differences.’

Further assessed oral sulopenem against amoxicillin/clavulanate (Augmentin) in a cohort of 2222 adult women with uUTIs. Participants were randomly assigned to receive either treatment for five days. The trial's primary endpoint was the overall response rate at the test-of-cure visit. Sulopenem achieved a response rate of 61.7%, while amoxicillin/clavulanate showed a rate of 55%, with a treatment difference of 6.7%. 

In the amoxicillin/clavulanate-susceptible group, clinical success rates were comparable, with sulopenem at 77.3% and amoxicillin/clavulanate at 76.7%. Microbiological success favored sulopenem at 75.2% compared with 66.7% for amoxicillin/clavulanate, indicating a significant advantage. Both treatments were well-tolerated, with less than 1% of patients discontinuing due to adverse events, and no serious adverse events were reported for sulopenem.

Together, these trials underscore the efficacy and safety of oral sulopenem as a promising treatment option for uUTIs, particularly in populations facing antibiotic resistance.

Leaders from the UNC Special Pathogens Response Center (SPARC) discuss how they plan and carry out biopreparedness trainings to address any serious individual episodes of highly infectious patients as well as addressing larger outbreaks.  

Biopreparedness Exercises Look to Address Highly Contagious Disease Outbreaks

, spotlighting individual medical institutions and infectious disease (ID) programs. Today, we spotlight the University of North Carolina (UNC)’s Institute for Global Health and Infectious Diseases (IGHID).

UNC’s Special Pathogens Response Center (SPARC) is one of 13 centers across the US responsible for building regional preparedness. This means these special centers interact and coordinate training with other health care facilities, such as local and regional hospitals, to aid them in dealing with individuals with high-consequence ID (HCID).

“While there is not a standardized definition for [HCIDs] or special pathogens, expert consensus defines these as novel or reemerging infectious agents that are easily transmitted from person to person, have limited or no medical countermeasures (such as an effective vaccine or prophylaxis), have a high mortality, require prompt identification and implementation of infection control activities—for example, isolation, special personal protective equipment—and require rapid notification to public health authorities and special action,” said William A. Fischer II, MD, director of emerging pathogens at IGHID.

 Examples of these would be Ebola virus, SARS-CoV-2, and Middle East respiratory syndrome coronavirus.

“We lead regional trainings to better prepare other health care facilities, because we're trying to make sure that anywhere a patient can present, they are prepared to identify and initiate safe delivery of care and then transfer that individual to a regional treatment center,” Fischer said.

Part of the training is to ensure any necessary quarantining of individuals is done in a way that minimizes the risk to the greater public. To prepare for big events such as the FIFA World Cup in the US next year, national readiness exercises have been conducted this summer for HCID threats.

 and are led by the US Department of Health and Human Services’ Administration for Strategic Preparedness and Response. Specifically, they tested a coordinated national response to an HCID scenario.

In June, UNC's Medical Center SPARC team participated in Tranquil Passport, and conducted a test using a mannequin that was supposed to have a HCID. This training exercise focused on "interdisciplinary coordination among pediatric specialists, infectious disease experts, and transport teams. The simulation also included a mock surgical procedure within the biocontainment unit, allowing the team to exercise infection control, communication, and clinical workflows required to safely perform emergency interventions in a high-risk environment."

UNC's Tranquil Passport Exercise

"The country is in much better shape right now than they were before we did this exercise, because now a lot of us in this country know how to handle this much better," said David Wohl, MD, professor of medicine in the Division of Infectious Diseases at UNC.

As individuals with HCID will not know about these special pathogen centers, these trainings between the centers and the regional hospitals are critical, explained Wohl.

“We really have to prepare this network of regional hospitals,” Wohl said. “One thing that's really important to understand is infectious diseases that are emerging and reemerging are becoming much more common and frequent—and they're becoming bigger in scope.”

He referenced the West Africa Ebola outbreak of 2014 to 2016 that infected over 28,000 people with the deadly disease.

 Wohl said this is massive compared with previous large outbreaks, which were thought of in terms of 100 to 200 people being infected. 

As part of their experience, both Fischer and Wohl have been in hot zones in Africa treating patients who have had Ebola and Lassa fever. They have set up field treatment units during outbreak settings to isolate and care for patients. Their work in Liberia has directly informed emerging pathogen response and preparedness in North Carolina.

Although Ebola has not had an impact in the US, Wohl used mpox as an example that has seen outbreaks in the US in recent years. 

Fischer said the concept of specialized medical centers is not a new one.

“These centers of specialization…parallel what's been done for trauma. They parallel what's been done for sepsis, what's been done for ARDS [acute respiratory distress syndrome],” Fischer said. “If you can develop expertise at specific centers and then educate and increase awareness among all providers, we can then identify individuals who might have a special pathogen, get them to the center of specialty where that specialized care can be provided, and we know that model works.”

Biopreparedness Exercises Look to Contain High-Consequence Infectious Disease Outbreaks

UNC’s Benjamin Smith, MD, provides some insights into the logistics and steps behind these complicated and serious missions.

Transporting High-Consequence Infectious Disease Patients to Special Treatment Centers

, where we spotlight individual medical institutions and infectious disease (ID) programs. Today, we spotlight the University of North Carolina (UNC)’s Institute for Global Health and Infectious Diseases.

When the call comes in that a patient with a suspected high-consequence ID (HCID) needs to be transported, UNC’s Benjamin Smith, MD, is contacted. Smith is a board certified emergency medicine physician and the medical director for Carolina Air Care, which is a specialty transport program for UNC Health. Their fleet has multiple rotor-wing aircraft and ground ambulances.

In addition to Smith being contacted in such a scenario, leadership at the UNC Special Pathogens Response Center (SPARC) at UNC Hospitals is alerted. Whoever is on call at that moment, including 

William A. Fischer II, MD; or David Wohl, MD,

 will work with Smith and other local and state health agencies to assess the patient based on suspicion of symptoms or tests and whether they need to elevate the seriousness of the transport. If they deem the patient may have an HCID, then several steps take place to prepare for transport and to receive the patient.

Within UNC’s hospital, they need to set up a biocontainment area, which likely involves moving some patients out of the space. Laboratory technicians set up a specialty laboratory so they can run tests for the patient.

And as with any important project, communication is key and members of the emergency department become involved.

"All the people who are involved in emergency management, emergency preparedness, and emergency response, they'll know what's going on,” Smith said. “They'll help coordinate. That includes our communications department, but it's really communicating within the hospital to the other people who are there working—but also the community at large—if needed. I imagine this would be something that would probably make the news, and so [it's about] making sure that our community understands that we're here to keep them safe.”

“Behind the scenes, we'll start coordinating logistics of what providers [are involved], and nurses will be at the bedside to care for the patient when that initial phone call comes in,” Smith said. “We also have flight watch, which is our communication center for air care. Air care will also be looped in to start coordinating logistics of moving the patient.”

If it is a ground transport, Smith’s role is to go with a crew in a chase vehicle, providing support and oversight to the team during the transports. He will have personal protective equipment (PPE) just in case he needs to administer clinical care at the bedside.

Ultimately, UNC’s teams have the training and on-the-ground experience to handle such serious situations.

For example, earlier this summer, UNC's SPARC team conducted 

 for a patient with HCID using a mannequin. This training exercise focused on "interdisciplinary coordination among pediatric specialists, infectious disease experts, and transport teams. The simulation also included a mock surgical procedure within the biocontainment unit, allowing the team to exercise infection control, communication, and clinical workflows required to safely perform emergency interventions in a high-risk environment."

Smith participated in this exercise with not only a mannequin patient but actors as well. "We did like we do in real life, where we would activate our receiving area, activate our team air care, get our ambulance ready, and so we went and met the patient out at RDU [Raleigh-Durham International Airport] on the tarmac to transfer the patient from the back of the aircraft to our ambulance,” Smith said. “In this case, [we] also transitioned them from the transport ventilator to our transport ventilator, transported their drips to our drips, packaged the patient in our ambulance, and then took them back over here to UNC and did that patient handoff.”

“We can really take any patient with any illness from wherever, and we bring the expertise of UNC out to that patient, whether it's at a referring hospital or on the side of the road,” Smith said.

Transporting Patients With High-Consequence Infectious Disease to Special Treatment Centers

Joseph Eron, MD, provides insights on their international reach as well as some of the important trials they have been involved in, notably the PURPOSE studies involving the landmark trials around lenacapavir for HIV prevention. 

UNC’s Global Laboratory Attracts Trial Researchers, Discoveries Around HIV Prevention and Treatment

We are continuing our new series, Media Day, where we spotlight individual medical institutions and infectious disease (ID) programs. Today, we spotlight UNC’s Institute for Global Health and Infectious Diseases (IGHID).


UNC’s Global Clinical Trials Unit (CTU) is a research unit deeply involved in HIV prevention and treatment. The CTU facilitates research across the NIH Clinical Trials Networks and supports innovative trials in various locations, including the southeastern US, southern Africa, and Southeast Asia.

Specifically, within the unit there are research sites in North Carolina, Malawi, and Vietnam. Joseph Eron, MD is the codirector of UNC’s CTU, and he notes communication between the US and global sites as a key component of their work despite the several hour time differences between the eastern time zone of the US and those countries.

“To get us all together is a challenge, but we do it, and people are very dedicated…They really believe in the mission of our group, which is to improve the health of the people in North Carolina, and improve the health of people globally,” Eron said. “And what we're learning, which I love, is that what we learn globally, sometimes really impacts people in North Carolina.”

He notes UNC's work in the PURPOSE study, which looked at HIV prevention. CTU was involved in the Purpose 2 and 3 studies that looked at the HIV PrEP agent, lenacapavir, which was 

 earlier this summer. Their results showed very high efficacy in preventing HIV.

“I think it's going to change the way HIV prevention is done worldwide…PURPOSE 2 was done here in the US and South America, and PURPOSE 3 was done exclusively in the US, and focused on women at risk,” Eron said.

“In the studies that were done, it was nearly perfect. In fact, in the studies done in Africa, there were no transmissions recorded,” Eron said of women being studied using lenacapavir. “The studies done in the US and in South America, in transgender people, transgender diverse people, and men who have sex with men, there were just a couple of infections out of thousands of people in the study. So really, quite amazing.”

Lenacapavir has been studied and approved for protection up to 6 months, but Eron notes the agent is currently being studied for protection up to a full year.

In addition to his work on the CTU, Eron serves as UNC’s division chief of infectious diseases and he finds both roles are interwoven.

“I think the roles complement each other; because part of my job as the division director in infectious disease is number one is to recruit very talented people in infectious disease,” Eron said. “A lot of the talented people want to combine clinical work with research. So having that role as a recruiter for infectious disease at UNC is a big plus.”

UNC’s Global Clinical Trials Unit Attracts Researchers, Leads to Discoveries Around HIV Prevention and Treatment

The University of North Carolina’s (UNC) Institute for Global Health and Infectious Diseases Director Myron Cohen, MD, discusses the uniqueness of the institution

The University of North Carolina (UNC)’s renowned Institute for Global Health and Infectious Diseases (IGHID) was established in 2007 and brings together the university’s 5 health science schools with a particular interest in infectious disease.

“The institute was formed in order to bring together and create synergy among all these different parts, and that's proven to be true, and then to create opportunities that would otherwise be difficult,” said Myron Cohen, MD, the institute's director. “And in particular, [we wanted] to build infrastructure in North Carolina and in other countries so that investigators could pursue their goals with much greater ease.”

The institute’s roots are in research, and it offers opportunities around the world. They work in 45 countries.

“We have fairly large infrastructure in Asia, Africa, South America, and North Carolina, so we can help direct investigators from all of our health science schools and the rest of the campus to the best place to achieve their goals,” Cohen said.

In addition, he says the institute has large regulatory and grants management teams that help investigators more easily navigate potential research avenues and secure grants.

The institute secured $323 million in contracts and grants during fiscal years 2020-2024, conducted over 200 studies in 2024, and published over 1000 papers that same year.

In addition to research, UNC offers top-tier trainee and fellowship education. They have 35 postgraduates, physicians, and fellows and have trained over 750 fellows since 1998. The institute has also won 50 awards.

The Institute's Director Myron Cohen, MD, discusses the uniqueness of the institution and its ability to offer research and clinical opportunities globally as well as play a significant role in public health.
